L2FH Rotation 7 successfully performs 27th surgery in South Sudan
PANO - Vietnam’s Level-2 Field Hospital (L2FH) Rotation 7 at the United Nations Mission in South Sudan (UNMISS) recently performed a successful surgery for acute appendicitis with a rare anatomical variant. This marked the 27th successful surgery conducted by the unit in the mission area.
The patient was admitted with right iliac fossa pain. Preoperative diagnosis proved challenging due to atypical clinical manifestations. An abdominal ultrasound suggested appendicitis accompanied by terminal ileitis and increased bowel peristalsis, symptoms that are easily confused with other gastrointestinal conditions.

Lacking an abdominal CT scanner, doctors relied on close clinical monitoring, abdominal wall reaction signs, and subclinical results to make a timely surgical decision. This crucial intervention prevented severe disease progression and dangerous complications for the patient.
The surgery was led by Major, Dr. Phan Van Hau, Head of the hospital’s Surgical Department. During the operation, the team discovered a retrocecal and recurved appendix hidden by ileal loops. This rare anatomical variant, combined with the limited surgical field of the McBurney incision, complicated the dissection and treatment. However, thanks to the professional experience and seamless coordination of the anesthesia, nursing, and surgical teams, the inflamed appendix was accurately identified and safely removed.
The surgery was completed after 60 minutes. The patient remained conscious with stable vital signs and was transferred to the ward for post-operative care.
Major, Dr. Phan Van Hau noted that despite the complex anatomical variant and limited imaging equipment in the field, early detection and a timely surgical decision were the key factors in successfully treating the patient and ensuring their safety.

The success of the 27th surgery reaffirmed the professional capacity and surgical emergency response of the staff at L2FH Rotation 7.
